Vital Statistics

As per an Executive Order from the Governor's office, January 1, 2003, the following must be adhered to in order to obtain a Vital Statistic record:

Anyone requesting a Vital Statistic record must satisfy the following information

  • Exact name as it appears on the Vital Statistic record
  • Exact place where event occurred
  • Exact date of the event
  • Contact Township Clerk's Office for other specific requirements

Moreover, the only people entitled to obtain a Vital Statistic form are as follows

  • Subject person of the Vital Statistic
  • Authorized family members (parent, legal guardian, spouse, child, grandchild or sibling
    of legal age
  • Funeral directors
  • Court ordered requests
  • Government agencies for emergent circumstances

The Office of the Howell Township Clerk maintains records of all births, marriages and deaths that occur in Howell Township. Listed below is information on vital records.

MARRIAGE LICENSE:


Cost: $28.00

Where should you apply for a marriage license?

A couple must apply for a marriage license in the municipality where either spouse resides. If both spouses are non-residents of New Jersey but plan to marry in New Jersey, the license must be obtained from the Registrar of the municipality where the marriage is to be performed and is only good for use in that municipality. A New Jersey license may never be used outside of New Jersey and a license issued in another state may never be used in New Jersey.

MARRIAGE CERTIFICATE:

Cost: $25.00 each

To obtain a certified copy of a marriage certificate, either write, e-mail or go to the Registrar's Office (Office of the Township Clerk) in the town where the event occurred. When requesting copies by mail or e-mail, please indicate in letter full name of each partner (including maiden name where applicable), parents' names, date of marriage, place of marriage including town, photocopy of either partner's driver's license and your telephone number. Checks are payable to the Township of Howell.

DEATH CERTIFICATE:

Cost: $25.00 each

To obtain a certified copy of a death certificate, either write, e-mail or go to the Registrar's Office (Office of the Township Clerk) in the town where the event occurred. When requesting copies by mail, please indicate in letter exact name of deceased, date of death, place of death including town, parents' names, your relationship to subject individual (with documents showing proof), purpose for which the record is needed and your telephone number. Certification form required to be filled out. Checks are payable to the Township of Howell.

BIRTH CERTIFICATE:

Cost: $25.00 each

To obtain a certified copy of a birth certificate, either write, e-mail or go to the Registrar's Office (Office of the Township Clerk) in the town where the event occurred. When requesting copies by mail, please indicate in letter exact name of person, date of birth, place of birth including town, mother's maiden name, father's name, your relationship to subject individual (with proof), purpose for which the record is needed and telephone number. Checks are payable to the Township of Howell.
